Medication at School
Parents, please fill out the top portion and have your child’s physician fill out the bottom portion. The medication name, dosage, and time must match on both sections. If you have any questions, please feel free to call us at 303-347-4525.
Parent/Physician Medication Permission Form

The medication must be delivered to the school office by a parent/guardian. Please do not send the medication in with your child for the safety of all our students.

At the end of the school year a parent/guardian must come in to school to pick up any unused medication.
